tl;dw: Build your own house, buy land, buy furniture even hire a musician to live in your own house. You can have a kid (I'm so adopting an Orc kid as Elf) and you have to raise him (haha yeah he can play with the monsters outside).
Hearthfire will be out on the Xbox 360 on September 4 for 400 Microsoft points. PC/PS3 versions will likely be released a good month later, similar to the Dawnguard DLC.
