I'm using the following guide:

To install FreeHDBoot, and then MCBoot. I am able to successfully boot into FreeHDBoot, but when I try to launch any program from the menu, including uLaunchELF, I get a black screen.

I have reseated the HDD and I don't think its the issue, its a healthy drive and Free HDBoot is working. It's connected via RGB to a PVM but I don't think thats relevant since I can see the PS2 Menu and Free HDBoot menu just fine. This PS2 is chipped with a Modbo 5.0 chip if thats releveant, but I disabled the chip and nothing has changed. Which reminds me, is there an ISO I can burn with uLaunchElf so I can just install Free MCBoot that way?

Thanks for any help!

Does the FHDB/FMCB configurator work? All added options in the menu are just forwarders to other homebrews (on MC/USB/HDD according to configuration...) and using a premade disk image is a bad idea due to how the APA partition table works

Yes, you can get a premade CD version of uLaunchELF (https://drive.google.com/file/d/0B78Jh8sKWrBOelZZcEZOMlZmR0k/edit), extract the FMCB installer on an USB drive (mbr/fat32) and plug it in, boot the CD, run the boot.elf (newest wLaunchELF) included with the installer deep into the folders, go to the HDD Manager and reformat the disk, then run the installer... and install it :)

Some chips just conflict with FMCB, but Modbos shouldn't be and in fact they are commonly used to run FMCB on the non-softmoddable superslims (caveat, there are multiple things called Modbo, all being modified clones of the Matrix of variable quality)

Thanks for the reply, I tried the latest FHDB and that just froze during the boot, so I tried burning the ISO but my PS2 just says "Can't read disc". I know it can read discs as it booted my copy of MSG just fine. Do I have to use a specific app to burn the disc? I just burned it using ImgBurn.

Do I need to burn it to a CD-R instead of a DVD-R?

Yes, CD and DVD images are incompatible, as far as the PS2 is concerned

Thanks, got it working. Just found out that unfortunately you can't use a PS1 memory card for this exploit, so it'll have to wait till the weekend unless I can get FHDB to launch apps and then just run it that way till then.

You don't need a memory card at all if you're going for FHDB (just because it shares the installer with FMCB) - and a clean install will come with a working wLaunchELF

I'm going to try reinstall FHDB again and trying the configurator like you said. Maybe the shortcuts aren't linked? I'll report my findings in a few minutes

So it turns out I didn't disable my modchip, when I hold start to disable it, I am supposed to hit the reset button once to reboot into a disabled mode, where before I was powering it off. It is now launch apps off the HDD, this will work until I get FreeMCBoot up, thanks!

Had identical problem as OP (installed fhdb, plugged USB but screen went blank no matter which option I selected, including configuration option), turns out all I had to do was disable modchip. Modchip my ps2 has was Matrix, and found youtube video demonstrating on how to switch it off: press and hold start on controller before you turn on console, then turn on the console (while holding down start button still) and matrix screen will write "disabled". then I simply restarted console, booted into fhdb and everything on menu works. hopefully will help others.
