    Test the card bud. Did you buy it off a 3rd party seller? Can you post a link to the item on Amazon?

    I just need to ask, but are you sure its Fat32 and not exFat32, also have you run the usual checks on the card to make sure it's not a fake one?

    Does the vita work as normal without the adaptor in? Also it could be that the sd card is corrupt or fake, use H2testw to test it and try a different sd card in the adaptor.
