From a disc, with disc2app iirc, and iso, wud with jwud tool, but I'm not sure if you need nus packer too.

Wii U USB Helper is missing a lot of keys. Also "that title key site" is missing some. Anyway, needing keys got basically deprecated with this: https://gbatemp.net/threads/3ds-wii-u-titlekey-generation-algorithm-leaked.566318/ (which is why NUSspli does neither contain a key database nor uses "that title key site": It simply uses math to get the key from the title ID).
