Homebrew [help] What is root cause of this error?

Hi, all
I play vWii on my WiiU recently and find a error while I launch usb loader gx or the homebrew channel. This error does not always happen and it is ok after restart WiiU.
Do you have this error like this picture? And what is root cause of it?

Are you launching with haxchi?

Thanks for your kindly reply. I hit B button and power button of WiiU at the same time. It will launch Wii menu directly and I think I do not have any chances to launch haxchi.
