Make sure you have a CLEAN/UNPATCHED ROM!
Use xDeltaUI.exe to apply the patch.
Select the Patch File (patch.xdelta, included in this archive), specify the Source File (the game you'd like to patch, and the Output File (the destination folder, don't forget to add the .nds at the end).
Click the patch button, wait a few seconds until a dialog pops up on your screen, and place the new .nds file on your SD Card.
