And no, you don't need to name your games as <game_ID>.Name of your game.iso anymore.
Using a router ? => DHCP + NetBIOS is recommended setting.
Using a crossover cable ? Static + IP is recommended.
Did you enable SMB ?
Settings > ETH start mode > Auto ?
Are you stuck on the menu screen ?
Did you share correctly PS2SMB folder so it can be accessed over the network ?
EDIT: Do you also need to set the ETH prefix path?
I saw that in the settings aswell. It's set to <not set>

Let it as <not set>. Useless for most config.
by using file sharing, windows doesn't allow you to share files if you do not set a password to login. you need to edit a registry key for it (i don't know each one)
i know that i was never successfully sharing a folder on windows without a login

Did you also create a DVD folder in PS2SMB folder ? You need to drop your *.iso in it.
