Hacking Wii returns to main menu when a game is selected

I recently ran through ModMii to update my Wii, and everytime I run a game in any USB loader - I get returned to the main menu. My Homebrew channel is using IOS58, so I can't figure out what the issue is.
I ran the IOS58 installer, and it fails. I'm running out of ideas. My Wii is on 4.2 firmware

You will have better luck getting an answer if you post a system check, and if you reinstall ios 58 it will turn your homebrew channel upside down and you will have to install hbc again.
