I can get to OFW by launching from hekate but from power off I can’t boot OFW by pressing power with vol+ and vol- pressed. It doesn’t actually bother me but it doesn’t seem right so I wanted to check if I should worry about it or just keep going. Is this an indication of a future problem or a poorly installed chip?

Pressing the volume button(s) works with picofly/hwfly units.
I don't recall with which firmware revision they added this feature though.

Volume - should also work.

Booting to OFW is done in software so if you can boot into CFW, then your hardware is fine.
No wires are connected to the volume buttons anyway.
I once had to repair a switch that had liquid damage on the volume buttons but you should notice that if you can't adjust your volume either.

[100% STOCK OFW]
ofw=1
icon=bootloader/res/ofw.bmp

[SEMI-STOCK (SYSMMC)]
pkg3=atmosphere/package3
stock=1
emummc_force_disable=1
icon=bootloader/res/stock.bmp

You can try these two see if stock boot.

It ended up being something with the fuses. I was able to unplug the chip, boot once normally, then plugged the chip back in and everything has been working as expected
