Seems they changed the default setting for the half pixel offset, even using an updated version this should work by right clicking on the game and create custom config, and then change the half pixel to the option No.2
In any case, did you reported this on the PCSX2 issue tracker?
Confirmed here, I'm also testing this great 50fps patch and the game looks incredible. In my case I can see the issue you're talking about, is like the water waves are blinking, but in my case the issue is a bit lower, I can see the waves blinking but on the video you posted seems worse than...

For 2D games, maybe is also useful to use XBRZ filter instead of upscale every single texture, if you have a decent PC the upscale work with this kind of filters is very nice. I use it on epsxe/PCSX-R and this game or any other like Alundra looks stunning.

I completed the game like 2 years ago. Did you remember if this game had chapter select or something like that? Because in that case maybe I can share my save file with you. But I'm currently travelling and I'm not returning to my home til friday evening.
Sounds like the bug is because of your PCSX2 version instead of the graphic pack itself.
After PCSX2 v. 1.7.5663 the way the HD texture packs are handled changed a bit, that's the reason if you disable the mipmaps is working for you, since in some cases this mipmap disable fixes this kind of...

Since most of the graphic packs on this forum are broken on the latest PCSX2 versions after 1.7.5663 I was wondering if anybody knows of to build a PCSX2 version but without a specific commit (This one: https://github.com/PCSX2/pcsx2/pull/11010) because of this commit everything broke suddenly...
So, basically disabling mip map allow the older texture packs to work right? I've tested a few ones (Metal gear solid 3, Tales of the abyss, sotc, dragon quest 8, etc) with this setting disabled and eveything seems fine.
These raindrops on screen sounds very nice. I remember one of the first videogames I've seen with this effect "Metroid prime 1" on samus helmet and wow... the atmosphere was incredible
