Open create_xbox_drive.ps1 in a text editor (that shows offsets like Programmer's Notepad) and look at each of those lines to see what it is trying to do. It will probably be the same exact problem in each instance because the offsets look redundant.

I wonder if it is because you need to run the .bat as admin. I looked at the script and it is whem the format command is run when it errors.

Edit: just saw it confirms admin. Try the gui version instead.

why not just download the offline system update and install it? i can see you selected option A which is simply building a new hdd - let the offline system update do it for you
