Hacking Modded Wii Button Input Problem

I have a problem with my 4.1 modded wii. The only game that I really play on it is brawl and I have some problems while playing it. When playing with any controller (Wiimote w/ nunchuk primarily) sometimes the button inputs aren't being registered. For example, I will hold the shield button and the shield will not always come up. If the shield does come up, it will flicker on and off as if i'm pushing the button over and over again. This is also a problem with charging smash attacks. Smash attacks will start to charge, but will initiate by themselves even if I did not release the buttons. When I play with a wavebird, button inputs are inputted twice so it is unplayable. Playing with a wired cube controller is better but it is still wonky.

I'm running the latest version of USB Loader GX, d2x v10 on ios249, and DiosMios v2.10 installed. I also have some old cios installed on other ios's. Could that be the problem? I have no idea what is wrong.
