Yes, you need the Sega CD/Mega CD bios for the 3 regions in the system folder.
They should be renamed as follow:
eu_mcd1_9210 --> bios_CD_E.bin
us_scd1_9210 --> bios_CD_U.bin
jp_mcd1_9111 --> bios_CD_J.bin
Maybe, you are missing the bios file. You need to download the bios system card v3.0 and renamed it to syscard3.pce.
Than you need to place it in the system folder (retroarch/system/).

BIN/CUE works perfect for me. Not problems at all. Sounds great. Plays great. Full speed at all time (at least the games I have tested so far).
The standalone GenPlus GX can support ISO/MP3 (encoded at 128 Kbps). Not sure about GenPlus GX for RetroArch.

mwalk doesn't even work in FBA for PC. So that report must be wrong.
You need to load the child rom mwalkbl (Michael Jackson's Moonwalker (bootleg)) to play that game.

It works "fine". I just tested it.
It has GFX glitches but they're present in the Official FBA for PC too. So nothing that RetroArch can do about it.

The Gamecube should be able to load ALL CPS-1 games without any problem. The biggest CPS-1 rom is Slam Masters and only has 6.28 mb (6.435 KB).

I can confirm about the sprites scrambling in Carrier Airwing. It didn't hang halfway in the 1st level though. I tried 4 times.
The Irritating Maze is a driver problem in FBA. It doesn't work neither in the Official FBA for PC.

I can confirm this issues as well but they're present in the latest official release of FBA so I doubt the RetroArch team can do something about it.
You can report this issue to the official FBA team or wait till RetroArch team port Mame 0.72 to RetroArch since it has no issues with both games.
There is only 5 games in FBA that use samples:
Donkey Kong (uses samples from dkong)
Donkey Kong Junior (uses samples from dkongjr)
Galaga (uses samples from galaga)
New Rally X (uses samples from rallyx)
Rally X (uses samples from rallyx)
So, must be something else. I haven't tried those...

Changing the sample rate to a higher frequency involves more processing for the CPU.
So change it from 44.1 Hz to 48 Hz to achieve steady 60 FPS is out of question.

The "tinny" sound that you are hearing in the last release of RetroArch is probably because the sample rate that Genplus GX is set now.
If you read the change log of RetroArch 0.9.8.2 it says that they change the sample rate from 44 hz to 48 hz (44.100 hz to 48.000 hz to be more precisely)...
USB Loader uses by default the cIOS in the slot 249. Modmii installs cIOS base 56 in that slot (I think d2x v6 base 56 if I'm not mistake).
You need to change it to slot 250 (since that's the one base 57) in the options before start the game.
