It's because it annoys people, like me. I don't enjoy getting second-hand lung damage from others and the coughing associated to it, so it's good that it's not "enforced" on us by it being censored.

I'm not sure if it's censored in germany. I still see smoking in movies and tv shows here. Maybe it depends on the age rating.
