If you think Dark Souls is the hardest game, you are wrong. Streets Of Rage is much harder. And it's not Dark Souls, it's Demon's Souls, that is a "spiritual" predecessor of the series.
Are we talking actually hard like requiring a LOT of skill or artificial difficulty hard where it's just mostly RNG?
I wouldn't call Serious Sam games actually hard for example since it's just artificial difficulty, you die in 1 shot and you get swarms of enemies that take tons of hits to kill and that's coming from the person that back in SS2 era was ranked 2nd in multiplayer maps for the entire 8 years the servers of that game were up.
As for hard games that require actual skill, main games are definitely the entire Kingdom Hearts series with all their superbosses requiring great reflexes and frame-tight dodging, especially since on highest difficulty you are just level 1 meaning even random enemies can one-shot you, let alone bosses!
After that I've finished the harder of the Etrian Odyssey games all on highest difficulty if offered and also various other hard RPGs. If we move past RPGs, it's probably King's Field (binged the series recently) but the difficulty there comes from old mechanics. Dark Souls and the whole Souls series fixed a lot of issues K'sF had so they are much easier in my opinion, especially with dodges in DS having stupidly long invincibility windows (like seriously, 26 iframes is ridiculous when other games I play such as Monster Hunter only have 12 or KH having 8 and enemies are slow AF compared to those two)! There's some others I'd say are "hard", but can't remember many past the ones I mentioned, hardly ever had difficulty in games after all.

sometime games become very hard at the end, but it's the normal difficulty progression. or if you enable 'hardcore mode' you can't call the game hard, it's your own choice to do that.
But if I completed them, I can't say they were really hard.
Like Zelda2 which can be considered hard by some, I completed it few times on NES, so it wasn't that hard.
Hard for me means I can't advance, and usually I drop playing them (like Furi on PS4)
real hard games, such as Ghost & Goblin, or La Mulana, were only completed by cheating so it's not fair to give them as the hardest completed game.
Maybe a lot of NES games could be considered hard, but I don't remember if I completed them, most often I reached far levels, but died before the end.

New generation games (since Snes/PS1?) are not real hard games, they are difficulty dosed to make you feel it's hard, and you can often change the difficulty yourself. or they are hard but on purpose to make you die thousands of time (soul series?) but have infinite lives and auto-save checkpoints, new mario game have infinite lives too, etc., but not hard in "you only have 3 lives and 3 continues !" arcade type game we had 30 years ago and you had to start over from the beginning ! I can't count how many time I tried to complete Shadow warrior on NES...
There are some epic hard and considered very good games (like contra/contra3/ghost&goblin,etc.)
old video games were hard by themselves, mostly because of the control, and limited movement or actions, limited features (save ram, password, etc.)
now, you can jump, move while in the air, come back, dose the jump distance, etc.
old one you had pixel precision jump without air control.
you completed ghost&goblin without cheating? I always admire people who could do that!
I do for completing all games with limited lives and continues I never managed to do myself.
I have played so many games but I don't know how many would qualify as "hard". That is a really subjective thing.
I just finished The Super Mario All Stars version of Lost Levels. I don't think it was that bad. Right after that I 101% DKC1. Hard but not that bad.
Ninja Gaiden 2 on Mentor was definitely a really big challenge. Mentor isn't even the second hardest difficulty setting.
Coordinating and keeping a clear head to beat all the the Destiny Raids on Hard Mode was really challenging too.

Also I wasn't saying that was the hardest game ever (you can still be wrong with your statement). I was saying none of the other games I've played were that hard in comparison.

I will say this, God of War 1 is a joke even in God Mode, beaten it a good 12 times. On the other hand, Titan Mode on 2 is one of the VERY few games that have made me ragequit a game for good and it was at the dumbest part, it wasn't some boss or tough enemy I had trouble with, it was the fucking annoying hands when getting back from the underworld after Zeus kills you, after about 200 tries where they'd barely touch me but still one shotted me I went all "fuck this shit" and stopped playing! It was then that I stopped playing GoW altogether....

For me Ares on God Mode (Very Hard Mode) and the Kratos Illusions make me rage a few times before i actually beat them and the final boss on 2 in Titan Mode is not as hard as Ares but still hard in its own right.

For Kratos clones I just spammed the O supermove you get after maxing out the blades (the AoE leap, that thing is broken AF) along with some dodging. Ares was ok I guess, but if you know his moves he's pretty easy honestly.

I've always played DKC games since I can remember (literally can remember my pre-kinder days and I was already playing DKC) and of course those are games that I finish from time to time in a couple of hours.
Zelda Ocarina of Time on the other hand, I finish it when I was like 10 years old, I know it's not a difficult game, but have in mind that I barely knew english at that age. I finished ALL THE GAME with the aid of a Spanish-English dictionary, literally translating word by word all the dialogs. I guess that helped me learn English.
So the game was hard because this language barrier thing, but I overcame that obstacle and I finished the game.

Only thing drive me crazy is all these new horror games nowadays is its so dark, all time. I can't even play them. I understand they trying to create spooky but be creative a little, not just darkness with pop out scare scenes.
