I would replace IOS249/250 with d2x with a newer version (use base 56/57, not 37) but other than maybe updating IOS58 I don't see anything wrong with that.

The Hermes v4 are old but you can just not use them.

I also have hermes v4, and never see any need to update to 5.x
It worked fine with v4 so I kept them.

Like said, no need to update or delete them, just keep them and don't use them.

Edit:
ahh, yes, I forgot to check your syscheck for issues with drive launching.
Look at what jdbye said below. you don't have pimpmywii nor dark/cioscorp, so your softmod is fine and not affecting the disc drive.

Your IOS249 is a bit outdated, but I don't see anything there that would cause issues with disc loading, have you set Priiloader to force IOS249 on the system menu/disc channel? That's the only software thing that could be causing issues since you don't appear to have DARKCORP or similar installed.
