Most likely one is the game and the other is an update or DLC. Is one way bigger than the other?

On the site I was looking at, it was a cia for dragon quest vii, the total file size is 1.4gb and the two cias were 999mb and 400 something mb, so not dlc

That's just because TWLoader is comprised of two parts, the 3DS-mode frontend where you select the games, and the TWL-mode backend (nds-bootstrap) which actually runs them.

Highlight both with your unzipping program (winrar) and click extract here. It spits out a single file for me. Look online how to merge cia files, should be a single command of the above doesn't work
