If you rent a game and then dump it to get the header, after you return the game someone else might do the same or use it online. And so that header won't be private anyway.

Theory; what if it's an offline game? Could you rent it because the game will technically won't ever show online?

Makes sense. It would never show up online on any server and so it supposedly couldn't get flagged even if someone used its header somewhere else.
But if two ppl rent it to do the same (as in extracting the header to use in roms online), it could still happen. So even though it might be unlikely, it's not 100% safe.

This is completely false. Formatting with an internet connection is fine. In fact it removes any downloaded (but not installed) updates.

Sometimes you need an internet connection to format too. It removes linked nnid

Better be safe than sorry. Some people claimed that formatting when connected to the internet did actually update their systems.
