Anyways, I structured mine in a specific way to reduce redundancies in questions (e.g. if you already said no to using SX OS/LayeredFS, there's no point in filling out whether or not you used them online). But arguably the time saved/lost is minuscule so...onto my other issues with this form.
- The "other" category might complicate the answers provided. Rather than saying a simple yes or no, someone would have to interpret how to log particular answers when its not logged as either. And since I'm so far the only one that handles this data, I kinda don't want to make more work than necessary. Like this might be useful in identifying additional causes not apparent in the base questionnaire, but it might be better to have the option for people to add comments to their Y/N answers.
- The version of SX OS is not a yes or no answer.
- I wanted to keep separate the methods of AutoRCM in case specific ones are safer than others
- Although I can see why you replaced "Username" with "GBATemp name", not everyone has a GBATemp account (crazy, right?). For some, it just might be easier for them to stay a lurker and use a Gmail account to contribute to the cause.
Again, I'm not trying to put you down, I honestly find this very touching someone would go through the effort to re-design my questionnaire into another Google Form. But unless the above issues are addressed, I would prefer not to use this. If you wish to add me as a collaborator regardless, I wouldn't mind either.
