OK, I've used hdparm on a WD 3.5" IDE/PATA - I then formatted it, converted to WBFS using WBFS MAnager, then formatted again to NTFS and then back to WBFS.

MY question is: will such a drive in an external enclosure using hdparm keep the spindown information saved somehow, or do I have to use hdparm each time I format to a format Windows can read?

The drive is NOT a Passport or equivalent. Just an internal OEM 320GB 3.5" PATA/IDE drive in an external shell.

Also could the external case's USB controller cause the spindown?

It depends. Some drives keep the settings by default. Others require the "-K 1" switch. Some won't keep it ever. You won't know until you try.
