No, as far as i know, you can not rip WiiWare sprites..

Well, you not alone on that, I've never done it by hex either. GAME.DAT seems to be the entire game, so that's where the sprites would be.

Because people made tools for those. Real rippers can make their own tools and understand how to find the images in hex.

just unpack the WAD with WADTool, then unpack *.002.des (00000002.APP) and you get all files, they are png and jpg, so it's no work to rip them ^^
for Dr. Mario, it's an easy format, not much work to extract files out of this Game.dat
but what do you mean by "my pc could handle it" ? use a prog like Winhex and don't use common ROM ripping Tools.
