I got tired of the bad earth djinn icon so I took Isaac's son's in-game portrait and made it into an icon for AKAIO.

Oh and the skin that it is previewed on I made about a week ago because I wanted a simple skin. If anyone wants it, just ask and I'll upload the files.

Open the ROM in something that can edit the header info, like DSBuff.
Usually there's a section of tools to export/import the icon. In DSBuff it is the top button (a folder) next to the games icon.

From the AKAIO How To:

Per File Icons

Specific icons can be assigned to specific files in AKAIO. Make a standard 32x32x15bpp BMP icon for the file and name it the exact name of the file, but with a .BMP extension. For example, for the file "Bob's homebrew Game.nds", you would use a BMP properly formatted BMP file named "Bob's homebrew Game.BMP."

Make sure to place these icons in the same directory as the file you are assigning them to.

Nice skin.

Some small tweaks, since AK Edit doesn't put everything exactly right:
Under [folder text]
x = 13
y = 4 -> 1
w = 160
h = 16
color = 0x0000
It will align the directory text on the bar at the top.

under [start menu]
x = 164
y = 0
itemX = 10 -> 12
itemY = 82 -> 75
barLeft = 8
itemHeight = 11 -> 12
itemWidth = 74
Will (mostly) line up the Start Menu.

I know AK Edit shows the text as being off but if you put it on your card as is they are in the correct spots to line up right.
