Hardware Need to format a MicroSD

Just got a cheap 4GB microSD in a flea market near my home.
It looks like it's original.
It's an 4GB LG microSD HC,
It even states behinf the label some numbers and the "Made in Korea" legend.

When I plugg it on my lap, its regonized immediately, but it says that it's empty and it needs to be formated.
Of course, this won't work, because it won't let me do a quick format, and making a regular format stops JUST before completition and a window appears telling me that Windows couldn't complete the format, make sure eveyrything is plugged correctly bla bla bla (or something along those lines, since it appears in spanish, yes that strange language)
And putting the SD in my phone is the same, it's recognized immediately but it won't let me format it.

How can I format it succesfully?

Ok, so, of course, I'm using a MicroSD adapter and it doesn't have the lock on.
Any suggestions on how to fix this?

My guess is the SD card is damaged or counterfeit.

Or maybe the adaptor is damaged?

Nooo... the adaptor isn't damaged, i used the ones that I use on my Wii and 3DS, both of them works on their respective consoles.
I've also tried the CMD thing, and, when reading the disk's properties (on CMD) it CLEARLY states that ReadOnly is OFF.
