Which installer? Cause im not sure anymore.
cIOS38r14_installer.zip run from the HBC. Then use the online install if you have wi-fi or put IOS38-64-v3610.wad in root of the SD card and use "Wad Install".

The file i downloaded just has the folder with a boot.dol meta and icon then the IOS38-64-v3610.wad im still a tad bit confused.

put the folder in the apps folder on your sd card and use NUSD to download the IOS38v3610 and pack to wad or if you have wifi just run the installer from HBC and choose network install

"boot.dol, meta and icon" go in the HBC apps directory ie. \apps\cIOS\boot.dol
"IOS38-64-v3610.wad" goes in root directory of your SD card ie. \IOS38-64-v3610.wad

Run the installer (boot.dol) using the HBC and choose Wad install. Your IOS249 now at rev 14.
