I have an USA cia of Rythm Heaven Megamix on an EUR N3DS that only boots with "Enable region/language emulation and external .code" and the .txt file inside the locales folder, if i update to the latest hourly release of luma it doesnt work anymore, is it something known to be broken?
The option to do that in the latest hourly It's called just "Enable game patching" and it also hints at custom romfs, so did something changed and i need to do something else to make it work?

When is luma 6.7 going to come out as I use the nightly to get everything working but now SaltySD does not work now.

The problem he had is that the folder structure changed from Stable luma to nightly and he had to move some files.Nightlies have issues with some titles like the one you mention and others too and is a kinda a hit and miss situation with the region/language setting for some games, as I personally don't use hourly builds since they are for testing and not general use.
I use a Japanese n3ds and rely on region/language emulation a lot, since most titles will default to japanese text, without this feature. Tbh, stay were you are on 11.2 and wait for a 6.7 official release, as I'm currently on 11.3/luma 6.6 and staying there for now.

You mean payload from HBL? Where can i download just that from please?Assuming you have a9lh and the latest nightly Luma, use the 11.2 payload and rename it for 11.4. It should work.
