All the tutorials I've seen suggest installing patched IOS56 and IOS57
versions via the D2X cIOS installer to 249 and 250. However, when
using those in USB Loader GX, my USB drives are never mounted and I
end up with the 'USB not initialized' message.

When looking at SysCheck's output, I noticed that only IOS58 lists USB
2.0, so I tried using IOS58 as the 'loader IOS' in USB Loader GX, and
with that one, it was able to list the files from my flash drive. But
loading an actual game on IOS58 failed still.

So I figured I'll install a patched version of IOS58 using cIOS into
slot 251. I did that, and using IOS58 as the loader IOS and IOS251 as
the game IOS, I was able to load the two games I was using for testing
(Rythm Heaven Fever and Super Mario Galaxy 2).

Now, my remaining problems are the following:

    - The games are quite slow to start; in particular, SMG2 takes ~17
seconds to go from showing the three planets representing the three
saved game slots, to allowing me to select one of them.

    - The above only works with one USB flash drive and one USB HDD out of
two flash drives and three HDDs I tried; and the one HDD it does
work with was a loaner. Any idea why it might not work with the
other two HDDs? I'm using FAT32 file system.

Please find my SysCheck output here:

d2x v6 is very old. I'd recommend updating 249-251 to v8 and see if that helps. If it still doesn't try v10-beta52. (Don't use beta53, it's very flaky.)

    Hey GreyWolf,

I tried, with d2x v8-final, the following loader/game IOS combinations:

    249/249[56] "USB device not initialized"
    250/250[57] "USB device not initialized"
    251/251[58] "USB device not initialized"
58/251[58] Same as before: works with one flash drive only, loading is super slow, SMG2 save menu still takes approx. 15 seconds to start becoming responsive.

With a HDD, 58/251 it gets to listing the games, but trying to start one just quits to HBC.

    So next up: d2x v10-beta52.

With d2x v10-beta52, I can use 249/249 as loader/game IOS versions to use my hdd, AND it is fast to load! So I think I'm good!

Should I still try 250/250 and 58/251? I'm too afraid to change anything now...

You only need 250[57] for a few games (Just Dance series, Call of Duty series, etc.)
I'm not even sure what requires 251[58]. I've never had to use it. :)

    Backup cIOS,

Some old guides, also suggest or include wads with 245 to 250 patched.

Also, if i recall correctly, ios58 dont need to be patched, nothing uses a Patches IOS58, Nintendont [USED] not quite sure if still uses, and HBC use IOS58 [CLEAN]

    Also, @OP:
You should update your CIOS to D2X v8, those were the latest stable Cios.
