What are the dimensions for a full (front&back) EU DS box cover (in cm) ?

I made a CycloDS box cover some time ago, but I can't find the template I used

Anyway here's the dimensions of the box: 14.5 x 12.6 x 1.5 cm
I'll try to find you that template (it has the dimensions and necessary DPI)

it's for printing a real (not custom) cover but it has to be resized first.
