The commercial isn't much to go on but I think this is a game where you play a wannabe superhero, with a coal shovel as a mask, on his quest against evil Irish Catholics to gather up all the rubber diaphragms propagated throughout the world. I accept that there is no evidence of Irish Catholics actually in the commercial itself, but I thought I'd jump to that stereotypical conclusion given the birth control device he is holding at the end.

EDIT: After viewing the cover art properly, I'm now not so sure? The anorexic girl on the left might work, but I can't get the guys in the background with anuses for faces to fit my theory.
