ROM Hack Converting 4th gen .pkm files to 5th gen .pkm files

I got an R4 cartidge with Pokemon Platinum and HeartGold ROMS and an original copy of Pokemon White, and my goal is to transfer my pokemons from both Platinum and HeartGold to the White version.

I uploaded all the .pkm files i want to transfer to White, and im planning on transffering them trough IR-GTS, but my question is, how do i convert 4th gen .pkm files to 5th .pkm files (and make them also look legit in-game)? Is there any possible way to do this? I've been searching google and asked on so many forums but still couldnt solve this out. I currently got Platinum && HG/SS Pokesav,Black/White Pokesav (V0.06C RELEASED BY COM), and PokeGen(5th gen).
