Hacking Can't install cIOS_fix.wad - error

Can't get cIOS_Fix to install, keep getting an error. Using a PAL wii with v3.4E with HBC Beta9 and IOS36. Can anybody suggest anything to help?

3.4 can't directly install custom wads.
first use wad manager 1.3 to install ios16
then use the patched wad manager (pointed to ios16) to install other wads
just install the latest rev8 version, that iosfix is old
