In-memory patches are applied by canoe, the SNES emulator.
Known issue with canoe (and early SNES emulators) - use Retroarch/Snes9x.
If you have a save state, it will use that as a replay, otherwise you'll get the effect you described.
In-game saves (SRAM) != Save States. Pick one or the other.
