Bringing this up again,
Still have this error and can reproduce it. Happens only in docked mode. Happens when I go into the album (the XCI loader), regardless whether I close the game before. Also happens when I play psnes and a game tries to save (by itself, regular game saving). Both does not happen handheld

Does the error require a full reboot of the console? or is it a automatic shutdown?

Only way to avoid it when docked to a TV, is undock, switch game titles or hb and the redock?

It shuts down the console, so reboot with dongle needed.
Only way to avoid it is take the Switch out of the dock, yes. PSNES is unplayable docked since it crashes on every save.

I've had the error once.

This is how it looks:
Error Code: 2345-0021

An error has occurred.

Please press the POWER Button to restart the console. If you are unable to restart the console, hold the POWER Button for 12 seconds to turn the console off.

If the problem persists, refer to the Nintendo Support Website.

support.nintendo.com/switch/error

It didn't automatically shut down for me. 12 seconds wasn't required either, pressed the power button to shut down, pushed it once more to restart.

Same here happened today...
Error 2345-0021 with SXOS 1.5 docked switch, entering album. Samsung 128Gb EVO SDcard (sold by Amazon so I would hope genuine).

"happens also pretty regularly when using ftpd with big files (over 200mb)." Same experience, ftp every time multiple cards. I just tried 1.6 same issue.

it also happens on other cfw with the newest SDfiles, I am surprised that the problem is still not fixed

Problem occured yesterday eventhough I reformatted my SD card on Windows (docked, after exiting a GBA emulator and entering Album). I also checked if Archive was unchecked on both sxos and switch folders so that's not the problem either..

I'm getting this error...
I'm creating a game on Brew.js and it only happens in docked mode.
Right in the middle of the game after so many objects are drawn to the screen.

The same with me when using FTP. It is strange that I can still transfer data despite the error message.

SX OS 1.9 - Switch in Docking Mode
400GB Micro-SD FAT32

Edit:
The latest build of nx-hbmenu sould solve this problem.
