it works.
if you bought a new card, mostly likelly its dead on arrive..
if you can find another console o try it, or check if its propertly inserted...
the number on board doesn't mean anything (at least no one found out exactly what it means)
are you sure you inserted a SDHC card? if you are trying a 2gb or less card it may not work... (or the oposite in case the firmware is the old one)
try another micro sd card (always format it with panasonic formatter (or sda formatter)

I mean it doesn't work on ds in ds mode.
It works correct on gba, gba sp and ds lite in gba mode, but doesn't work on ds lite in ds mode...
I tested new one with passcard and r4i gold and got black screens but old one and supercard mini sd works correct with passcard and my ds lite.
I tested it with 4Gb sdhc card and tried panasonic formatter as you adviced.

ds mode was removed on firmware 1.74
so no.. its not defective.. you can flash the old firmware (1.72 or less) but keep in mind, NOR will most likely stop working on that firmware, you will also loose SD-HC support, and you will need a non HC card (2gb or less) (in case you don't have one, you will not even be able to flash the new firmware back)
Thanks, I tried 1.72, 1.71 and 1.66 and I got the same result: two black screens.

As was already said, the new EZ Flash IV does not work in DS mode anymore. There is no way to restore DS mode. The archaic DS mode available with the cart is dead. The DS mode was outdated crap anyways. This is the second thread where people have apparently still been using slot two solutions for slot 1 in 2015, and it baffles me when dedicated slot 1 carts are so convenient and so, so, so cheap.

I have 3 slot1 cards, yes it so, so cheep. I don't planed use ezflash with ds. Of course I will use this card with gba only

DS mode is still useful for dumping games and saves with slot 1 & 2 combo. Dumped at least one hundred carts that way

For the new EZ flash IV, it supports only the gba roms on GBA, GBASP and DS lite, not supporting the DS mode. If your EZ flash IV works properly in the GBA mode, I think it's a good one, instead of a fake one as you said.
