Im looking for a PC game where I could play any type of character I want. Where I'm not forced to follow any quest line if I don't want to. Where I could go out, kill the shit out of stuff, and return home to say, a companion. Or a lover. Or whatever. Something easy to lose 100+ hours of my time. Preferably something either fantasy, or in some sort of medieval setting. Nothing modern. Open world would be great but isn't required. Nothing MMO either. I'd like something that looks anywhere between oblivion and Skyrim Special edition, graphically.

So open world sandbox game? I was gonna suggest Just cause 3 but there is no "Loving" in that game. Sound more like Grand theft auto 5 i guess, there is hookers, and you basically do way too much with almost no limits. I watch twitch streamers play online with others who just randomly kill each other and team up and do bank heist and other dumb stuff. Retro city rampage was a favorite of mine, it has a free play mode where you basically just do whatever you want in the open world, but is very small world considering the game is about 40MB and looks like a nes graphics, but there is mods i think for custom characters.

What you asking may be a bit of a stretch. Not many games are like you requested, custom character, non mmo. Most games that do have custom characters now and days are encouraged to be MMO, and hardly have Lovers to return to considering is fictional and not the focus of the game. That game would have to combine open world freedom with dating sim and custom character development which has yet to exist all at the same time. Good luck finding that game, but Grand theft auto and Maybe saints row 4 are close to it.

It does exist. It's called Skyrim. Which I already have. Maybe I should let off on some of my requirements. It doesn't have to have a lover system as any type of main focus. Just so long as the gameplay doesn't make me feel like the world is empty.

Errm... This isn't exactly what you asked for but there's a good chance you'd enjoy Stardew Valley. It's open in the sense you're dropped onto a farm and left to do as you please, albeit in a relatively small world when compared to your Elder Scrolls and AAA epics. You've also got a Harvest Moon style romance system so you can get acquainted with the locals. I suppose the world's size does benefit the game in meeting your criteria though, every part of it feels alive. You feel as though you're a part of the community, which is pleasant. It's also 40% off on Steam atm, so it's as good a time as any.

I would suggest Fallout, but you said nothing modern. Something that doesn't fit your requirements at all, but I've had a lot of fun with, is The Crew. It's an open world racing game where you can do anything you want. You can drive across the USA, or play through the story. If you want to try something new, you can try Battleborn. It's like Overwatch, but free. (Only online multiplayer, no private match)
The Dark Souls series might fit your needs. You play as an undead who wanders the world aimlessly trying to find and kill any bosses. There's no lovers, no companions, only you. You do get to find new people around the world though who will go act as merchants in the game's hub world. Be warned, it's brutally difficult.
You could also try Lords of the Fallen. It's like Dark Souls, but much easier. I personally prefer Dark Souls, but this might be more fun if you hate dying. I haven't played much of it, but it feels much more linear.

You know what? Let me put it like this: suggest me a game with an open world, that isn't modern in any way (setting wise) where I can go and kill shit for a good two hours and then go and do whatever I please. Murder like a psychopath, guard a caravan, be a bard, or whatever else. Just as long as I have game pad support.

Hooooooow about Windward! It's a randomly generated open... Ocean! You own a ship which you can upgrade as you progress through the region. You can hunt pirates, take on quests, or just be a nice person and go about trading in friendly tides.

OH MAN would i suggest the FUCK out of Dark Souls so badly but sadly no lover. However it does take place in the time period you like and it is very good in the graphics department (have you seen videos of Dark Souls 3?) and has lots of enemies to kill (or kill you).
You are not required to go in a linear path but there is a story and i my self have spent 1000hrs+ on DS 1+2 because there is many ways to play the game (i.e. Character builds and more)

then you will want either GTA V, SK:SE, The Witcher series, or some other game i cant remember
