Yes, you can use an app like WiiMC to play videos off an HDD even if it has games on it too.

It should be noted that the Wii cannot play HD videos, and it even struggles with some standard-definition videos encoded as H.264 (MP4), depending on how graphically intensive the video is. WiiMC is great if you're playing Xvids though.
MKV videos are typically encoded using H.264 (same as MP4), which the Wii can struggle with depending on the nature of the video. If it's a standard resolution video with a high bitrate and a lot of action, you might run into some AV sync issues. If it's animated, you likely won't run into any problems. To minimize these problems, you'll want to go into WiiMC's settings and set Frame Dropping to "Always." The reason the Wii can struggle with graphically intensive videos is because the Wii just isn't that strong of a machine.
If you use Xvid, you likely won't run into any problems. Xvid is less intensive that H.264.
