Hacking ModMii failed to install necessary ios, cios

I posted this in another place; hoping I can get some fresh ideas...

Used ModMii (4.8.2) to fix my really screwed up Wii that had been previously soft modded. Current firmware is 4.1; selected the option in wizard to load 4.1. Get errors/return 2011 when trying to load the following WADs (step 5 in wizard).

Tried multiple times but same results. Installing IOS236 (step 3) was successful; confirmed that all of the following are physically in the "WAD" folder

I also tried running the syCheck output thru syscheck updater in ModMii. It tries to install the same IOS as above and fails. The output of sysCheck is:

according to his syscheck it's installed, so that's probably not the issue.

However, it's possible however unlikely he has one of the very first versions of "IOS236" installed which did not include the version patch, if that's the case then he would need to reinstall IOS236 using the latest installer. But the OP said he used the IOS236 Installer that came with ModMii v4.8.2 and that it successfully installed so I doubt that's the issue here.
