I'm only using Mouse + Keyboard for sims & fps, steering wheel for racing games and in every other case I'm playing with one of my 20+ gamepads (NES, SNES, MD, N64, ..., ..., GC, PS3, 360)

Yeah, but then the Wii U crashes when loading the next game (5.5.1 PAL-User here).
It's much better to turn your Wii U off and on again to play the next game. Those 20-30 seconds more are worth it to have everything working fine.
All your question can be answered by just having a look into those 2 folders, take any Wii U game.
Cover: YOU choose the size you want, whatever fits best (I like them a bit smaller than Wii U game covers so I chose 168x236 for NES games). Put it in the covers3d folder like every other cover...
You don't need the original ID's, you can name them whatever you want (6 letters). Like: [VC1234]
Just make sure your cover ID is the same as the game's, then put a background pic in you like.

Well, I didn't see it the past few days and wasn't sure, that's why I said ".. seems like there is indeed a new update (5.5.2 or 5.6.0 ?)"
I have no problem not having a 5.5.2 update by Nintendo ;)
I have a 128GB SDXC card by Komputerbay (33,00 €) and I used "HP USB Disk Storage Format Tool_v2.2.3" to format it to Fat32 because "Fat32Format" didn't work for that SDXC card.

It's my second day playing Wii U backups and I wonder if it's normal that you have to try like 3-4 times to make loadiine work. It freezes very often so I have to restart my Wii U like 20 times to play 2-3 games.
So, my actual question is - Is it really that unstable or am I doing something wrong?
