Hi,
I'm trying to make some modifications to some game which I have installed on an external USB hdd.
I managed to access the content of the game using cfwbooter + fw.img + ftpiu everywhere/

Is it safe to edit the files and store directly in the /storage_usb/usr/title/xxx/content folder?
Will I be able to launch the game later?
Probably I need syspatches first?

Yes it's safe. As long as you don't touch system titles in nand, you should be fine. Be aware that a bad manipulation could always happen, so be warned that brick risks are still there.
