I just use NTFS for my Wii's Harddrive, and haven't had a single issue with it on CFG Loader or CleanRip 2.0, everything just works great, and all games play fine when ripped using either program. :)

Try using a different cIOS version. You need to install 250[57] anyway.

I may installed cios250 57 successfully now, but situation does not change.
And I also tried d2x-v6, but nothing changed.

Is this a hardware level issue?

The Japanese version of Xenoblade is a single-layer Wii disc, so your dump should be fine.
Only the US and European versions are dual-layer Wii discs.

Well that settles it then - explains why all the JAP rips I found were around 4GB's.

EDIT: A quick google search confirms it, (RVL-P-SX4J) Xenoblade Chronicles (NTSC-J) is a single-layer disc, unlike the western release.

Odd, I ripped it to FAT32, but USB Loader GX did indeed split the file into two WBFS files, worked fine when I ripped it, NTFS won't work on most USB loaders or other apps. Also dumped Super Mario Galaxy 2, Brawl and MK Wii, all of which use dual layered DVDs on FAT32. but again, it does split.
NTFS works fine on USB loaders.
